Adverse Drug Events
Excessive Anticoagulation on Warfarin
Any patient in the hospital being treated with Warfarin (Coumadin) with an INR level greater than 6. (A lower percentage is better)
Hypoglycemia in Inpatients Receiving Insulin
Any patient in the hospital being treated with insulin with a Glucose level less than 50 mg/dl. (A lower percentage is better)
Oversedation due to Opioids
Any patient in the hospital receiving narcotics who is oversedated, requiring medication to reverse the sedation. (A lower percentage is better)
Why is this important?
Administering medications appropriately and safely is an essential component of excellent patient care in the hospital.
