HealtheLife / Mammoth Patient Portal

Patients Age 0-11 Years

Parents/legal guardians may obtain proxy access to a minor’s portal account with the parent’s email address and their own HealtheLife/Mammoth Patient Portal account.
Parent/legal guardian must be a Mammoth Hospital patient and have their own HealtheLife/Mammoth Patient Portal account to access their child’s information online.
Parent/legal guardian access terminates when patient reaches 12 years of age.

Patients Age 12-17 Years
Minor patients may access the portal by creating an account with their own email address (not the parent’s email address).
No parent/legal guardian access to child’s online records from 12-17 years of age.
Parent/legal guardian access to HealtheLife/Mammoth Patient Portal is restricted due to California state laws protecting minor confidentiality in certain health circumstances.

To obtain a copy of your child’s unrestricted records please the Mammoth Hospital Medical Records Department at 760.924.4041 or email HIMS.Email@mammothhospital.com.

Patient 18 Years and Older
All patients may access their health records online by creating a HealtheLife/Mammoth Patient Portal account with a unique and valid email address.
Patients may grant proxy access to another patient’s records by completing the appropriate authorization.
The patient may terminate Proxy Access at any time by contacting the Medication Records Department at 760.924.4041.
